Member Toolkit for Promoting Health Checks

To help you spread the word to your coaches, parents, and players about completing Health Checks, LeagueApps has put together a communications toolkit below, which you can copy + paste from.

You must turn on Health Checks for a program in Program Settings in order for program participants to be able to complete them. Health Checks are available for registered participants to complete 6 hours prior to an event (game, practice, etc.) starting.

Email To Send Your Members Introducing Health Checks

To ensure a safe and healthy playing environment, we will be collecting Health Checks prior to each scheduled game, practice, or event. The Health Check includes 4 ‘Yes or No’ questions recommended by the CDC.

You will receive an email with a link to complete this self-evaluation of your current health 6 hours prior to each of these scheduled events. You can complete the Health Check by clicking the link in this email before you arrive to the event so there’s no delay getting started.

How To Answer Member Questions About Health Checks

What if I arrive at a game and haven't completed my health check yet?

While you cannot access a Health Check form more than 6 hours prior to the event, there is no time restriction after the event starts. So, you will be able to still complete the Health Check form even if you arrive late.

Can anyone attending the game fill out a Health Check form, even if they aren't a player or coach?

Currently, LeagueApps' Health Checks are not publicly accessible. You must be registered to the program's team as a Staff or Player to fill out the Health Check form.

Can I complete my Health Check in the mobile app?

While Health Checks currently aren't available in the mobile apps, this functionality is coming soon. Stay tuned!

Can Health Check form submission responses be edited after they've been submitted?

No, staff, participants, and organizers cannot edit a Health Check response once submitted.

Will I receive a confirmation email with Health Check submission results?

No, a confirmation notification email does not send after you completes a Health Check form. The Health Check status can be viewed by the participant in the form completion popup in the Member Portal. The coach is able to view the status of each of their players in the Health Check tab on the Event Details page for that event.
